You will require a Cuba Tourist Card (Tourist Visa) – you must collect this card before you travel. It can be collected from Cuban Consulate, travel agency or authorised visa application agency)

Cuba visa application form.
A valid South African passport. This must be valid for at least 3 months after the day that you leave Cuba and must have at least 2 blank pages where the visa will be affixed.
